01The challenge

Clinicians abandon a digital intake form in a large share of sessions, falling back to paper and creating a reconciliation burden downstream.

02Our solution

We shadowed eleven clinicians, rebuilt the flow around their actual sequence rather than the database schema, and validated three prototype rounds before development began.

03The result

Abandonment fell to 6%, average completion time halved, and the flow passed WCAG 2.1 AA audit on first submission.
